Liamgo Provider Insurance Disclosure Agreement

This Provider Insurance Disclosure Agreement (“Agreement”) is entered into between Liamgo LLC (“Liamgo,” “we,” “us,” or “our”) and the individual or business registering as a service provider (“Service Provider,” “Provider,” or “you”) on the Liamgo platform.

This Agreement governs the insurance responsibilities of Service Providers using the Liamgo mobile application, website, and related services (collectively, the “Platform”).

By registering as a Service Provider or continuing to offer services on the Platform, you acknowledge and agree to the terms of this Agreement.

1. Role of Liamgo

Liamgo operates as a technology platform that connects customers with independent service providers.

Liamgo does not provide insurance coverage for services performed by Service Providers unless explicitly stated in writing.

Service Providers are responsible for maintaining any insurance required for their services.

2. Provider Responsibility for Insurance

Service Providers acknowledge that they are solely responsible for obtaining and maintaining any insurance coverage required to perform their services.

Depending on the nature of services offered, appropriate insurance may include:

• General liability insurance
• Professional liability insurance
• Commercial auto insurance
• Workers’ compensation insurance (if applicable)
• Other industry-specific insurance coverage.

Providers are responsible for determining what insurance is appropriate for their services.

3. Optional Insurance Documentation

For certain service categories, Liamgo may request documentation verifying insurance coverage.

This documentation may include:

• Certificates of insurance
• Proof of policy coverage
• Expiration dates for insurance policies.

Submission of documentation does not create an obligation for Liamgo to monitor or maintain insurance coverage on behalf of the Provider.

4. No Platform Insurance Coverage

Service Providers acknowledge that Liamgo does not provide insurance protection for providers, customers, or service activities performed through the Platform, unless explicitly stated in writing.

Providers assume all risks associated with performing services offered through the Platform.

5. Liability for Services

Service Providers are solely responsible for:

• The services they perform
• Any property damage or personal injury resulting from services
• Compliance with safety and regulatory requirements.

Liamgo is not responsible for the actions or services of independent providers.

6. Indemnification

Service Providers agree to indemnify and hold harmless Liamgo, its affiliates, employees, and partners from any claims, damages, losses, or liabilities arising from:

• Services performed by the Provider
• Failure to maintain appropriate insurance coverage
• Violations of applicable laws or regulations.

7. Changes to This Agreement

Liamgo may update this Agreement periodically.

Updates will be posted with a revised Last Updated date.

Continued use of the Platform after updates are posted constitutes acceptance of the updated Agreement.
